剧情梗概：
拳手林健儿（吴奇隆饰）不甘受黑社会大哥阿勇（陈松勇饰）的摆布，拒打假拳，准备携家逃跑却被抓回出身警察世家的杨重（金城武饰）因行事莽撞杀死了线人，丢了公职又犯下杀人罪；一干天涯沦落人在火烧岛监狱服刑，股票大王吴世达（吴孟达饰）在狱中成为众人觊觎的目标，幸有酒鬼阿梁（赵自强饰）和林健儿出手相助，吴世达出钱令林健儿、阿梁和杨重在狱中得到照顾。不久，狱中贩毒集团大哥宫天保（楼学贤饰）向当年抓获自己的杨重寻仇，杨重却以德报怨在阿勇袭击宫天保时出手阻挡。宫天保安排人手报复阿勇，自此与林健儿、阿梁等站到同一行列。典狱长（黄秋生饰）欲从宫天保手中获得更大的利益，多次设计陷害宫等人，众人不堪忍受决定奋起反抗剧情版本二：林建儿（吴奇隆饰）是个拳手，为陈阿勇卖命打赌博拳赛，因想脱离其控制而遭追杀，为保己命而杀人，因而入狱服刑，陈亦同。除暴组干员杨重（金城武饰）在一次银行缉捕行动中，因开枪不当伤及无辜被撒职，判刑入狱。宫天保是个威风八面的大哥，人在狱中还控制着外面的黑道交易。典狱长古金伦是个热爱古典音乐的笑面虎，老李是票据犯，也是个多金的凯子，大家都想从他身上弄钱。林、杨、宫在狱中因互相照顾扶持成为好友，林更因替宫办事得利，宫使用特权帮他把儿子带到狱中相见。古设计骗李的财产，李受不了破产的打击自杀，被林救下，两人成为忘年之交并相约逃狱，却失败了，李被射杀林则惨遭古割断脚筋。宫在狱中地位渐为陈所取代，两人更因争夺毒品贩售权而起冲突，古主张以拳击赛胜败来决定，宫以跛脚的林出赛，暗地里宫、林、杨则策划着一项逃狱计画
